We ask that you take some time to consider this form before committing to fill it in. Please ensure that you provide as much information and detail as possible. There are no opportunities to save the form and return at a later date so the form must be completed in a single sitting. 

The more detail you can provide, the easier it will be for the team to review. 

What happens to my form after submitting it? 

Completed forms will be sent to the Student Reps inbox for the Senior Representation Coordinator to review and to ensure the complaint is in the scope of the Rep Misconduct Procedure and that this is the correct place to resolve the complaint. If the complaint is in the scope of the Rep Misconduct Procedure, the Senior Representation Coordinator will inform both parties, organise a panel to review the complaint and begin investigating the complaint if necessary.
